Maybe you didn't tighten down the pressure plate even enough and it became skewed and some fingers are pointing out.
I have seen it on the NG900 model with clutch cable and that the clutch pedal is vibrating from the uneveness of fingers hitting the throw out bearing. As soon as you push lightly on the clutch pedal, everything is smooth.
1/4" space sounds a lot though between fingers and bearing so I wonder if you have something else going on.
What does it look like when someone is pushing the pedal and releasing it with engine off? Does the bearing back off a 1/4" right away or does it only happen when the engine is started?
And can you see that it is in fact the bearing wobbling a 1/4" or a finger on the pressure plate sticking out?
Maybe you didn't get the bearing seated on the slave, but that kind of doesn't sound possible.
