I just use a crescent wrench on the back end of camshaft and a 14mm socket and ratchet.
Have not encounter one yet that didn't come loose.
Yeah those plastic guides does not like any rattle and roll in their old ages. Just lifting off the head can be enough to knock off the end of the stationary rear guide.
