I don’t have a solution, but I have a few ideas. Water boils at 212 degrees F at sea level, and lower temperatures at higher elevations. Coolant boils at much high temperatures usually over 265 degrees F. You did mention that you were using water. Boiling could cause steam pressurization which could exceed the pressure to be held by the cap. Here are some other common causes I can think of:
Low coolant level
Defective overflow tank cap
Failed thermostat
Clogged radiator fins
Loose or damaged radiator/coolant hoses
Air in the cooling system
Failed water pump
Defective coolant temperature sensor
Broken fan
Head gasket failure
Cracked engine block

You have addressed the fan issues and the thermostat and hoses, but that leaves other possibilities. Did you check your engine oil for any signs of coolant from a leaking head gasket?
