You are right, the sprocket attaches to the camshaft with the 3 bolts. The "cam arm" is used for service only and must be left unattached when the engine is assembled. Here's how the "arm" is used: a few months from now when it's time to adjust the valve clearances in your engine, you'll have to pull out the camshaft to get to the shims. In order to do that without mucking up the chain tensioner and cam timing, you will first set the crank and cam timing marks to TDC, then remove one of the 3 sprocket bolts and put it through the arm into the threaded "nose" of the sprocket. Then you can remove the other two bolts and pull the cam out, leaving the sprocket and chain hanging from the "arm", fully tensioned and timed. When you are done with the valve adjustment and have the cam back in, install the two loose bolts first and then finally remove the third bolt from the "arm" and install it. Ingenious, eh?
As far as assembling your "new" engine, I think you want to get the head on, then install the sprockets (set the cam timing) and finally the tensioner and then put the chain cover on, all before mounting the engine to the transmission. In that operation, the "arm" will just be along for the ride.
