Disc brake gurus?

One for the disc brake gurus.
Just put new front disc pads on my '67 (thanks Chip Lamb). I don't have any spring retainers so I just pulled the cotter pins, put the new pads in, replaced the cotters and off I went. Everything seemed fine for several stops then then at one stop light I hit the brakes I get a clunk grind grind grind grind.
Pull of the offending wheel (left front) and the outside brake pad had moved forward and was grinding on the caliper center. Brakes still work sort of but not a good situation.
So - what did I do wrong? The old pads were pretty flat so i made my best guess as to which pad had the thick part at the top - on the left front wheel I put the pad on the outside with the thick part up, inside has it on the bottom of course. Is this why it slipped forward? Or is there something that holds the pads in place?
