I have lived with this annoyance for 15 years and finally fixed it today. The issue was the that the shift lever pulled out too far and slipped out of the hole at the end of the gear shift rod. The disassembly and fix of the shifter was easier than I thought.

1.Disengage the shift rod at the lower taper pin on the universal joint allowing the shift lever to fall clockwise to the 6 o’clock position providing access to the square hole.
2.According to the service manual; press the inner plastic (black) turnstop (inside the square hole) with an arbor (I used a small flat headed screwdriver) and push the shift lever in to release the pin from the inner turn stop slot, turn the shift lever ¼ turn and it will slip into a secondary turn stop that allows you to pull the shift rod further out thus, disengaging it from the shift rod. I didn’t need to do this step as the shift lever would automatically disengage from the shift rod. I assumed that the plastic turn stop or small plastic ball was broken.
3.Remove the entire mechanism by unscrewing the large chrome nut.
In my case there didn’t appear to be any damage to the inner plastic turn stop spring or round plastic ball. But the pin was already in the secondary turn stop. A previous owner must have removed and replaced the shifter in the past. So, for the last fifteen years the shift lever and pin had never been returned to the first turn stop position. Had I known this I could have simply pressed the plastic turn stop and returned the release pin to the first position??? After a good cleaning and fresh coat of grey paint all is back together and shifting perfectly.
