Normal wear, just like any other engine part, makes the links more loose, which makes the chain longer (the individual links do not stretch, the length increases as they get loose). Usually you do not notice because the tensioner extends a click at a time (about 1 mm each) and takes up the slack.
It can be replaced without removing the engine, just pulling the camshaft cover. The old chain is cut using a "chain breaker" tool, and an open chain is fed in. You are supposed to replace it before the tensioner runs out to the end like mine did. (I was not expecting it at 100k)
At this point, I may have to remove the timing cover, replace the chain and chain guides, clean bits of plastic chain guide out of the sump and the oil pickup screen, worst case replace the oil pump.
