OUCH! Horrible oil analysis - 1996 SET bearing wear

I'd like to know if anyone can help me pinpoint the most likely source of these wear metals. Super high values for copper and lead, some silicon from a possible intake leak or bad air filter (was replaced recently). Car runs fine and gets pretty good mileage (29hwy, 24 city). Recent driving is lots of short 2-3 mile trips, plus some very tough mountain driving with heavy cargo (in short, extreme duty).
The oil I'm using is Rotella T synthetic, with about 1qt topup over 5000 miles. I think there's a small exhaust manifold leak, and some oil weeping off the engine block near the valve cover (front drivers side).
Here are the numbers from past three oil changes:
Miles on car: 130k 145k 155k
miles on oil: 5000 5000 7000
Iron: 9 9 14
Chromium: 1 1 1
lead: 5 40 244
copper: 3 50 320
tin: 0 0 3
all others normal/low
contaminants:
silicon: 3 13 23
boron: 3 7 11
Everything else is normal. I used some moly additive this last oil change.
The copper and lead point to bearing or bushing wear according to the analysers interpretation. Which bearings can these be? Crank, cam, both? Any suggestions on how to salvage this engine, or am I stuck waiting until it eats itself and I have to part it out?
