Looking at the huge amount of rust, lack of any antiseize on caliper sliding areas where the pad is contacting, I would say it is a combination of rust buildup on rotor surface creating a too hard (glazed) surface to break through and a binding pad that start to tilt sideways.
I don't so much see a tapered pad from outside to inside perimeters but more of a tapered pad from top to bottom as in picture 6.
If your car would have been an early 99 and not a 2002 and if the pads would have been tapered from outside to inside I would have said the wrong pads were installed using 819 since they have a "hump" on the inner pad that needs to be grind off (or use the proper EUR800) but the later 99 and up 9-3 have the caliper machined out to make room for that hump.
Also the difference in thickness between inner and outer pad as in picture 6, I see all the time even on cars here with no rust or binding calipers whatsoever.
It is like the friction compound gives up on one pad and it starts to wear down much quicker after a long time in service.
I haven't replaced many Akebono pads yet since I started to use them but several of them have had one pad almost metal to metal where the other 3 had 40-50% left.
