You could have described my car exactly - '01 9-3 (base model) with 157k miles, but it's had this problem for the 3+ years I've owned it. Haven't had any success getting it diagnosed, much less fixed, because it only happens on the first start of the day when the weather is warmer.
One mechanic did suggest trying a different brand of gas, which didn't help, but since it just started happening again last week, I think John's onto something with the switch to "summer blend" gasoline.
I also think that a minor air leak sounds plausible, but it'd have be really small, because my car has never thrown a CEL code for one. I do get this "wooo" sound on startup that I can hear with a stethoscope on the aluminum intake pipe to the turbo, but it disappears as the car warms up - maybe due to thermal expansion that stops the leak. Sounds like checking that clamp would be a good idea.
The other theory that I've seen proposed here is carbon deposits on the pistons that will absorb the initial spray of fuel from the injectors at startup. Once they're saturated, the car is then able to start on the second or third try. I've used various Techron and Seafoam treatments to no avail, but have not attempted a "steam cleaning" of the pistons with water into a vacuum hose.
Anyway, I hope this is in some way helpful, and I am interested in anything you might find out yourself!
