Blatantly non-Saab auto maintenance question

My family has a 1996 Dodge Grand Caravan with roughly 125000 miles on it. Today, as I was driving my siblings to school, the car stalled after I braked gently to a stop. The car started right up again, but when I attempted to shift it into drive, it bucked and stalled again. Some space opened up in front of me (thankfully everybody behind me was too...unaware to pass me, even though I had hazards on and was making "pass me!" gestures out the window) and I revved the engine high while in drive. I got the car to go about 600 feet before I had to come to a stop again--where the car, not surprisingly, stalled again. Now, it idles normally in park (and likely neutral, too). When I put it in gear, it fluctuates wildly (between 500 and 1500 rpm, I think). When I last got my oil changed, the mechanics suggested I get my transmission fluid changed relatively soon, but they said it wasn't immediate. However, I recently made a 914 mile journey (from Cape Cod to my home in Ohio) and that may have made the problem immediate. Assuming the transmission fluid is the problem, would changing it NOW fix anything, or is the transmission likely wrecked?
If anyone else has had this problem, and it was NOT transmission-fluid related, please respond also.
