In addition to true self-insurance where the company is liable for all lossess (though usually done throught a captive -- company owned -- insurance company), there are other ways to self insure.
The most common is the use of a deductible. The company will pay up to say $100,000 per claim and then the insurance company covers the excess. The premium is rated based on the size of the deductible.
The problem with GM and the like is multi-faceted. Its not just the premium being paid to insurers, but also the level of benefits, i.e., employee is entitled to two pairs of glasses a year instead of one (just doubled to the cost) or three dental cleanings a year instead of two.
