The contract may have some procedure for dealing with this type of event, so you can check there to make sure your bases are covered. I'd recommend that you immediately notify the contractor in writing of the items you are not satisfied with and why. This can be done with a cooperative tone, offering to work together for a quick resolution to the problem.
Naturally, the contractor wants you to be satisfied and become a source of referrals. The flaws aren't something he wants to happen, more likely the result of a worker who has a low level of awareness about quality, or just doesn't care. You mentioned health problems and everyone goes through times of distraction. Ultimately the contractor is responsible, but he needs to made aware of all your concerns so he can take corrective action. In a perfect world you wouldn't have to be the quality control department, but when humans are involved.....
