Getting in and out of the car while it's refueling (if you're doing the refueling) is indeed dangerous and is the cause of the static electricity (probably from seats) that has been known to cause fires at the pump.
I would think cigarettes would be dangerous too. However, my father in law has smoked while working at a full service gas station much of his life and has not caused any fires that I know of. If I had a gas station, it would certainly be a no-smoking place regardless.
I'm also curious about the story behind being asked to turn off your engine.
