If you're getting mildew/mold with the vent running, your main culprit is probably one of two things:
1. Your exhaust fan is undersized
or
2. Your exhaust pipe is clogged with dust - when was the last time you cleaned it?
Like Anders said, the underlying board may be too moist. If it's too saturated (you can't necessarily tell just by touching it as the surface paper may feel dry, but the core may be damp), whenever you try to cover it with a paint product, you're trapping the moisture in from that side.
Let's say the core is saturated right now. As it's exposed you're having residual moisture evaporate from the paper surface over time. When you cover it with primer/paint, you're impeding the evaporation. Over a short period of time, the paper gets moist enough that it literally pushes the primer/paint off.
Use a moisture meter and stick it in a few places to check the moisture level.
If it's too moist, replace it as it will eventually lead to mold problems - plus you can use the new mold/mildew resistant type of drywall that uses a poly/fiber covering rather than paper.
If it's dry enough, primer should be enough to hold the paint on, but use a separate primer - do all paints are usually never worth what you pay for them - especially in humid environments like a bath. Clean your vent piping and fan and check flow. Check the volume of the bathroom and go to a home center and compare it to fans rated for your bathroom volume. Sad to say that many, many, many bathroom fans are actually undersized as developers used whatever was cheapest for them...
