I have a 1998 4.3 V6. In 165K miles I have had about $3K of repairs: a/c, intake manifold gasket & radiator, alternator, water pump, and shocks. Still a very quiet and comfortable ride. I have no qualms about its reliability. If you need 4x4, the only legitimate offering is the hard to find ZR2 package as the standard S10 models were 100mm narrower in track and could not accommodate any wheel/tire upgrades.
For a 4x4 small truck, I would consider Nissan or even the GMC-Chev Canyon/Colorado which has an Isuzu I-5 engine. I'm not a fan of Toyota as I generally find them to be overpriced and underpowered but I understand that the pre-Tacoma SR5 models are solid.
