In the old days of cats, the ceramic substrate occasionally did disintegrate and plug up the unit, but I haven't heard of this lately. On cars made since 1996 however, the mandated OBD-II system checks that the cat is "working" through the use of the second downstream oxygen sensor. If the sensor value shows that the cat isn't working properly, it will cause the ECU to set a Check Engine Light, causing you to fail inspection if your state requires it. I imagine that close to 100% of cat replacements are done in order to pass inspection.
