For private cars, the insurance "travels with the vehicle" according to my insurance company (Liberty Mutual). The vehicle is insured and as long as you are authorized to drive it the car is covered damage, liability, personal injury. You could ask about being put on the policy as a driver, but it may not be necessary. Check with the insurance company and your employer.
On rentals, your own insurance "travels with the driver" and usually covers you in an accident, although this may vary with state laws and your policy. The add-on insurance the car rental companies offer is very expensive, although it seems to be OK coverage. Your credit card may also provide some insurance, either as part of using it, or as an add-on. With Amex, for example, you can get coverage for about $25 that's good for the term of the rental. That's pricey if it's only one day, but not so bad if it's for a few days. And if there's an accident your insurance company doesn't hear about it.
