This may vary somewhat from location to location, but...
My understanding is if you drive a friend's car and crash it, it goes on their insurance (insurance is tied to the car). It will affect their premiums, and being a good friend you will want to pony up and reimburse them somehow... So if you lend your car to a friend, be aware that there is the risk that if they have a collision, it will fall back on you and perhaps test that friendship! If someone is going to regularly drive the car, it is wise to put them on the insurance in case something happens, but it may increase the insurance cost of the vehicle (as appropriate wrt the risk, ie the guy's driving record). Basically, for an employer who is asking you to drive it, they should have appropriate insurance in place.
For rental cars, generally you are not covered by your insurance unless specifically noted. I have what my insurer calls "the Grand Touring Solution" which amounts to maybe $40/y or something on our policy and covers me for all rental cars (and other stuff). That's like a day or 2 of rental car company insurance (car rental company insurance is a total cash grab rip off because it does not factor in your good driving record).
