I no longer think this is a lazy or silly design. The engine is smooth and quiet. The water pump is replaced at the same time as the belt and the cost is less than one cent per mile (~$750 /90,000). Engines with timing chains are not certain to run forever. I'm not sure what can happen once their nylon chain guides wear thin but I can't imagine it being a simple or inexpensive repair after that.
P.S. look for YouTube videos (time-lapse) of Toyota techs replacing timing belts.
