Pulling an exhaust manifold is not difficult. Especially if it's been off before. As you noted, the only thing I'd worry about is breaking a stud, then all bets are off. I have replaced a broken stud with the head on the car, and it's not fun. I placed a mirror down below and worked using the reflection. I had to drill it and use an extractor.
If you take the manifold off, you may as well make the most of it. Don't just grind off the obstruction. Port match it, open it up, and polish the crap out of the inside.
I can't say weather it is enough of a performance gain to pull it. I did mine when I was doing a head gasket job. It's surely not as good as an equal-length header, but every little bit helps.
