I can't say whether or not the head gasket is necessary. It sure sounds like it is, though. You definitely don't want it to get so bad that it's leaving puddles under the car, 'cuz that probably means it's been sabotaging your engine for far longer! Sending a sample of your engine oil out for Used Oil Analysis would tell you pretty quick if you're getting even a small amount of coolant into the engine. I use H.O.Penn Machinery 'cuz the rep (Denis Adiletti, at the Newington CT office?) is a Saab owner & gives other Saab owners a break on the price.
I use 10k mile intervals, even if it takes more than a year, 'cuz it's easiest for me to remember. I think you could go a little longer on mileage if you wanted to (e.g., if you wanted to go by 1 yr intervals instead of mileage), but I'd try to keep it as close to 10k miles as possible. Your "stretches of cold periods that are in the teens" would make me nervous using 15W-50 year-round. The new M1 5W-40 would probably be what I'd use in your area. It'll last just as long as the 15W-50, so that's a call you'll have to make.
As for the 4Runner, you might want to consider if the warranty requires anything specific for oil & change intervals. Since it's not a turbo, you could use any premium full syn & easily go 10k miles between changes. But if Toyota's going to fight you WRT warranty claims, it may not be worth the hassle to extend your changes beyond the "normal service" interval during the warranty period. The same goes for using a viscosity that is way off from what's recommended.
Not that it would stop me. I'd use M1 5W-40 for 10k mi in both. Then, when the 4Runner was OOW I'd use UOA to see if I could go 20k mi. Of course, I'm NOT suggesting you do this. I'm only saying what I would do. :-D
