Unless Italian car insurance business differs very much from Dutch you needn't worry too much. With all the harmonization going on in the EC-countries in endless areas there shouldn't be too much of a problem in insurance. In most countries you can insure a car older than 25 years as a semi-classic, based on a percentage of the value and with a maximum permitted kilometres to be driven each year. My Sonett costs me ± €250,- insurance per year with a maximum of 7000 km. Last time I had the value estimated (7 years ago) it was ± € 9.100,-; with the current exchange rates that is about $ 11.900,- (Sonetts are relatively expensive at this side of the ocean). Look for an insurance company that is specialised in classic cars. You get a better deal for less. And I think you shouldn't worry too much about pollution control or not. If you have a 1967 car pollution regulations from 1967 will apply. If there were no rules in that year then there is nothing you must adhere to.
Like I wrote in the beginning, I don't know if Italian law differs much from the Dutch I am subjected to, but it would really surprise me if it did. As a plus, the South European countries have a much bigger passion for cars than here in Holland.
