For what it's worth, I had this happen once for none of the 'usual' reasons.  
I had trouble downshifting to 2nd for several weeks. Then at 3 AM on a
Saturday on my way home from a business trip, all I could get was 3rd and 4th
gear, then just 4th. My clutch suffered, but I got home, where I found my
taper pin connecting the shifter to the transmission had fallen out. What's
ironic is that these pins are next to impossible to get out when you want
them to come out...Steering fluid leaks had acted as penetrating oil!

If the taper pin gets sloppy, you'll have trouble with downshifts long before
the thing falls out.
