You do everything from below on the 4 cyl engine removing wheel and front portion of fender liner.
It is a tool that consist of a flat bar with an adjustable knee bar on and different sockets that can be installed depending on the hole in tension on different cars.
You might be able to manufacture something out of a flat iron bar and weld a 3/8" square on it in perfect angle.
You need to remove crank pulley to get access to and snake out the tensioner and I usually need to pry the engine over slightly to get the tensioner out between frame and timing cover.
