This statement is interesting:

Flush on a high mileage trans is bad news as it stirs up all the junk that's been harmlessly sitting in there. Drain/refill is the way to go.
Also, the "loose bolt" is a common failure on '01/'02 Volvos that use the same transmission. There's a bolt on top of the trans that looks JUST LIKE what you'd remove to refill the fluid. As soon as you remove it - BOING - you just wrecked your trans because that's not a fill bolt, it holds the 3rd gear band in place inside the transmission. Some tricky fishing around can sometimes recapture the band and save the trans.
