When I replaced our TB, I had cleaned it (not very dirty) and it was fine for a little while, then cleaned again, then fine for a year... then on a long drive in a remote area with stores closed and no tb cleaner in the car, it failed. Had to get it towed somewhere which meant someone else had to do the work to change the tb (even though it is easy). Not a bad idea to swap it if you have a known good one.
I really do not like how the upper o-ring fits in the groove... it is easy for it to slide out of the groove when you install the hose especially if the oring is older (good to replace it). I had used electrical tape as a quick fix to keep the ring in the groove, as even with a new TB, there were occasional codes especially if the hose blew off! btw, I had given the shop a new o-ring, but they must have lost it and reused the old one... I have had no codes though since taping the ring in place and replacing the hose clamp with a better one.
Also, you should make sure that limp home mode is reset properly whenever you get a code.
