It sounds to me like the heater core is the issue. I had quite similar symptoms a few years back and replacing the heater core ended up being the answer.
The tubing in the heater core is really really narrow, and it doesn't take very much for some portion of them to develop a blockage (especially after nearly 15 years).
Removing the hoses in the engine bay is to allow you to blow coolant out of the heater core before removing it to minimize mess in your interior, and also ensure that coolant from the rest of the system doesn't find its way inside (clamps don't always completely cut off the flow).
Finally, to try to bleed the system in case air in the core is the issue, start the car and let it get to operating temperature with the coolant cap off, and the heater on HI. Letting coolant circulate without the system being sealed and pressurized often lets trapped air escape (and is a simple approach).
