I probably misspoke when I called it a "conical washer" - the hub-center feature was initially (1976? and 1977) performed by a large washer, more like chamfered, held to the hub by the axle nut. The o.d. of that washer fit into the i.d. of the machined wheel, and that did the centering. I think that the washer eventually got replaced by the machined ridge on the hub.
As far as sizes, no argument - you are certainly correct about the diameters. I had trouble with 205/60x15s rubbing on the inside in the rear and no trouble with 195/60x15s. Likewise, I agree you can tolerate minor variations in offset, but as you change the scrub radius, you will start to impact straight-line tracking. I also find that all 195/60s are not equal as far as rolling radius - I recently changed from Pirelli's to Continentals of the same nominal size, and they were about 3-4% difference in radius (so says my odometer and speedometer, anyway).
