This stuff sucks, but at least the companies usually stand behind you. I've even had unauthorized charges on cards I was sent and cut up because I wasn't using them (keeping the oldest line open to keep the credit score up)...but I also work in e-commerce a bit so I'm a bit jaded:
Having just created an e-commerce web site that connects directly to a live authorization system, I can confirm, the CCV (3/4 digit) code is not needed. The address is not needed. The NAME (!?) is not even needed. These things get you a slightly reduced service fee (and I implemented them) for the transaction but the least you need is number and expiration, no more.
This doesn't mean you can use it anywhere with just number and expiration, just that thieves probably know where does check and does not, have people on the inside (especially at large department stores, etc.)
Funny fact: the address verification check blows up when you try to redeem gift cards that act like credit cards. The CC auth company's stand: don't check and you'll be fine, just turn that completely off around the holidays (!).
The state of security is sad, and up to the holder of the data. Amex is un-offshoring some of their operations because their subcontractor in India left a ton of info on a completely unsecured machine. This is why if you use an Amex in the past few months (like at the Wal-Mart self-serve line), you might get asked for billing zipcode, which was not part of that lost data.
In the case of MasterCard last year, they got it all out the backdoor, which is why many thousands of cards were pre-emptively replaced. Again, not a true hack, as hacking implies you had to circumvent something. In most cases the data is as good as in the glovebox of an unlocked car.
