Our lab (~90 pounds) did this about a year ago at age 9. There are a few surgical options; but not all of them are appropriate for a large, strong dog. We ended up going to the local Vet Specialists (our normal vet would not do it because of his size). He had Tibial Tuberosity Advancement surgery (google this), which bascially changes how the knee (stifle) works and changes the stresses. He was confined to a pen in the family room for several weeks and only went out on leash with a sling to add support to his hind quarters. I built ramps for the front steps and bought a nice collapsable one for my wife's car. It is easier to get a ramp positioned into the rear of an SUV (Escape then, Forestor now) than it is into the rear door of my 9-3 (my old 900 hatch would have been better).
At any rate, he is doing pretty well. He always had arthritis, so that is still there. He goes to a therapy vet for underwater treadwell twice a month. The surgery itself was around $3k. Yes, that's a lot to spend on a dog that age; but that's a personal decision not worth discussing here.
Do check out the Vet School angle as well. We considered going to Cornell University (very good Vet school); but it was a little further than we wanted to drive him while he was injured (about 2 hours from our home).
