It's time + temp.
If the engine is not heating up fast enough to a certain degree over a set time period, it triggers the code.
So when the thermostats gets lazy and doesn't close shut good enough or starts to open too early, that's when it happens.
I don't know the exact numbers but here in fairly warm CA, it takes about 2-3 miles from cold start until thermostat opens.
If I test drive a car and I see the coolant temp hovering around 75-80°C at cruising speed, I know the thermostat is bad.
