It could have been a casting or forging fault not detected by the manufacture or the factory. Being in QA, I know that sometimes hidden faults get through the system.
The fact that the shop had never seen the problem, and that no one else on the BB has had the problem, makes it seem likely to be an isolated incident.
One good thing is that you now have documentation of a transmission problem. If something else should happen to the transmission, even after warranty, you may be able to point to this problem and get some relief from SAAB.
