awful mpg from V6

What can I fix or change to get better gas mileage? Since I bought it about 6 months ago, I consistently get only 13 mpg on my 2001 V6 wagon.
I get an accurate computation by always filling till the pump shuts off and dividing those gallons by the miles on the trip odometer. I buy about 99% of my gas at the same Costco station in California.
I drive it with a very light foot watching the turbo to only briefly touch the boost zone when entering the freeway, etc. Otherwise, I stay in the lower third of the turbo gauge.
Could this be causing the poor mpg? There is still a constant, slow stream of small bubbles coming up in the expansion tank after I used Bars head gasket repair to stop a small leak that I think was in the head gasket. I don't lose, or have to add, coolant though.
The air filter looks clean.
The spark plugs look healthy with a light buff color.
Where is the fuel filter so I can check it?
