A small coolant leak in the heater core can lead to this.
Any smell of sweetness when the vents are on? (Some say coolant smells a little like maple)
Does the problem improve if you only let the floor vents run?
Any signs of coolant loss? (The leak could be so small that no coolant loss is noticed.)
