Can you explain what's going on with the cold starts?
No true test of the TB, but a common indicator is putting the car on cruise control and letting it roll along for a while. If the TB is on the way out, it will not keep a constant speed and will have to rev the engine back up every so often. That's not a detailed description, but search and you can find more.
When mine was on the way out, when I was trying to accelerate uphill, the throttle would cut in and out.
