I had similar issues on my 2008 9-5 aero with 125K miles while driving on freeways and at idle. Once in a while it would trigger two CEL codes and immediately go into limp home mode. I've partially disassembled TB got to the two potentiometers and cleaned them potentiometers and carbon track with electrical contact cleaner last year. I believe there is a guide or a walkthrough on how to get to them. There are electrical wires within the TB that could have shed its shielding due to constant heat soak cycles and may contact one another. It immediately rectified rough idle and no more power loss on freeways with very smooth idle and smoother acceleration. Issue never returned after 20K miles. As siwyjohn mentioned your issue could be a TB going south.
I'd also check all vacuum lines, their connections and one way check valves. Do you have PCV Ver No 6 installed.
