Look carefully at the lever - usually when they are failing there is a crack in the plastic where it goes around the shaft (the larger opening end). It may not have fallen off but may be cracked enough to prevent the allow the shaft to continue rotating beyond the stop point. You should be able to pop the lever off and snap the new one in place.
