I would think that if the tensioner failed it would probably still be in place and would rattle a good bit (front/firewall end of the engine) and you timing could be slightly advanced when starting at least (while running the friction from the camshaft would probably hold tension on the chain). So you may get a pause when turning the engine over like a cylinder had fired early or too much fuel/air mixture in the cylinder.
