Memory is foggy, senior moment, etc. What you need to do is take your grinder or dremel and cut a large slot in the shaft that you can stick a large screwdriver into. Preferably one of the Craftsmen tools with a square shaft that you can use a wrench or pliers on. Then you can use that to immobilize the shaft while you turn off the nut on the top.
I think the reason I remember pulling it out through the top was that my threads were totally frozen and I ended up cutting through and removing the washer that rests below the rubber spacer and cutting away the spacer. If you do that, then you can pull it up and out. I'd try the method above first, then if the thing still won't turn, you can hack away at the washer that is holding the thing in.
As a last resort, once the washer and rubber spacer are gone, you'll be able to cut the shaft off just above the shoulder that the washer rested on. But I don't think you need to do this. Have a look at Figure 2 on page 731-2 of Bently and it'll make more sense to you what need to be done.
