Holding the throttle open makes it just a little easier for the engine to spin.
While computerized engine management systems all tell you not to touch the pedal at startup, maybe in this case, at 15 below when batteries are weaker and oil is thicker, it's giving just that little bit of extra speed to the cranking that lets it successfully fire.
Oh, and t., make sure your engine oil is full up to the max mark, it will help seal the pistons and give you just a tad more compression.
