This may sound elementary, but to me, it sounds like the cone shaped reverse lockout mechanism in the bottom of the shift rod is out of adjustment. The shifter is what locks out reverse, not the transmission. Its a pretty common problem for the cone shaped bit at the bottom of the shift rod to no longer engage, and therefor not require the lever to be pulled up for reverse. When this occurs, 5th gear is really hard to find.
Adjustment is very simple. Take the three screws out below the shifter boot. Take the shifter out and remove the clip in the bottom. Spin the cone out a few turns and reinstall. There is a spec for the cone length, but considering wear in all the parts, it's best done just by feel.
5th gear is really hard to find with the trans on the bench. Thats because you need the shift rod coming out of the trans turned to the exact right spot before you pull it. The other gears are much easier to find with the unit on the bench.
