Step 1 - with the engine running, does the belt run in the middle of the tensioner pulley, or off to one side? It should run right in the middle of the tensioner pulley. Off even a little is the sign of a misalignment somwhere.
Next, do you have the original tensioner shock? That shock provides both the spring to tension the belt, and damping to handle load changes. As the belt load changes (for example, AC compressor clutch kicks in or out), the tensioner will move forward and backwards some.
When the engine is running, the center of the tensioner pulley - the bolt -will 'jiggle' back and forth a little. Usually less than 1/8 inch. If it's jiggling a lot, that's a sign of a worn shock. Next, watch the tensioner as the load changes - AC clutch kicks in or out. The pulley will move to a new location. If it moves smoothly and quickly to the new location, and then resumes the same small jiggle, that's a sign of a GOOD shock. If it overshoots the position and oscillates for a bit, then settles down, that's a sign of a worn shock.
I figure it's either a misalignment or a worn shock.
