I fully admit I could be mis-remembering this, but it actually largely depends on state law as to how much power the insurance company has in this situation. Of course, you always have the option of not filing a claim.

To my knowledge, in some states the insurance company can permanently brand the title with a salvage status, in some cases making it impossible to register the vehicle. Fortunately, where I live, the insurance company can not mark the title as salvage unless I consent to it.
