Yes, the front bracket may be cracked(see pic below to see minor cracking)), or the 4 bolts that hold it to the tranny are loose, or their holes elongated. I've seen them cracked through the bolt hole also. I say remove the radiator and the AC condensor to get to the front mount(radiator for sure, none of my 99's have AC). It won't tighten anymore no matter how hard you try. The thread on the bolt is only so long, and you are probably at the end of it, or everything is rusted. If the rubber on the front mount is worn, it also won't tighten anymore, unless maybe you pull the mount bolt and lengthen the thread with a tap and die set. I've also seen cracking in the body through the two bolt holes that hold the front mount to the body.
17mm for the bolt head from underneath, 18mm for the nut on top, and I've seen different sizes of sockets work on the nut. 5/8" is 16mm, neither the bolt head or the nut are that small.
