A few more things to check. I assume you have a timing light. Connect it to the main wire from the ignition coil and monitor the light flashes at idle. They should be constant with no drop outs. This will rule out any ignition problems.
With the oxygen sensor disconnected, does depressing the throttle switch make the engine run better? If yes, then the base mixture is too lean as depressing the switch makes the mixture richer. (If it has no effect at all then there's something wrong with the Lambda system. The pulse width modulation sound should change. It can be easily tested by jumpering the fuel relay on (without the engine running) and listening for the sound change when the switch is depressed.) Have you tried adjusting the base mixture via the adjustment screw in the fuel distributor?
Was the 20 BTDC at 2000 rpm, vaccum line disconnected? At idle it should be around 15.
