I'm not a two-stroke expert -- have only owned one, and that was a long time ago -- but have you ruled out the possibility of air leaks in the intake tract? Normally, if you have to use the choke for normal driving, it can be a hint that air is leaking into the intake stream without going through the carburetor, causing the mixture to run lean; using the choke (or 'cold enrichment device' on old stroker carbs) compensates by richening the mixture that IS going through the carburetor.
Surging is another symptom of lean running, so that's an additional hint.
The blue smoke might be a completely unrelated issue, or it might not, but it'll be difficult to troubleshoot until you've gotten the engine to run decently without having to use the choke.
