Car rolling while parked on hill

My e-brake has never been great. I always have to pull it to the very top of its range before I get any kind of engagement. My girlfriend can't get it that far so has never parked on hills, basically. I have got it seviced a number of times but it has only helped a bit (maybe one less ratchet needed to fully engage)
We just moved into a new house that has a steep driveway. I back the car in so that when it's left in reverse, that gear should pretty much lock it on the driveway with the e-brake not really needed, so I thought.
However, this doesn't appear to be sufficient - the car slowly rolls down the hill?!
First, the e-brake is shot I would think. But what would cause the car to roll forward down hill while parked in reverse?!
I do suspect that my clutch is not long for this world - could that be it?
