It appears to be the R.H. tie-rod end. The boot is torn, allowing moisture and dirt into the joint. You should definitely change it. The part should be relatively cheap, however, sometimes they're a bear to remove due to the threaded section seizing. Repeated soakings with PB Blaster starting a few days before removal may help. The alignment, specifically toe-in may be off after replacement. A good mechanic can measure the thread engagement of the old one before disassembly and get the toe-in pretty close, but it's always best to re-align if you care about tire wear. You may want to consider doing both sides if the L.H. side is starting to deteriorate.
