Sounds like it could be a couple things. First, timing chain and/or chain guides. Second, is the end result of sludging, though 99's not quite so much. both of these would account for the metallic sound. This is what I would concentrate on first.
For sludging the easiest is when you change the oil to stick a wire into the empty oil pan and swish it around, if it has thick ... sludge then you would have to drop the oil pan to assess the situation.
At that time you could also check the chain that goes to the balance shaft (I think it is called) many have been know to cut that chain and take it out completely. but You'd want to research that. But if that is the problem chain it is the easiest to get at.
If it is the timing chain, you can only get at it through the head. Timing chain issues are common. Could be as simple as a broken guide, sprocket with worn teeth or chain stretched past its prime.
